About the Session     Delivered by Dr. Farah Tabassum of Women’s College Hospital, this session highlights practical primary care clinical tools to support real-world care.

This is a clinical education session for healthcare professionals, including physicians, nurse practitioners, nurses, and interprofessional providers (OT, PT, social work).

Why It Matters   Led by the Alliance for Healthier Communities, this initiative has broad reach across Ontario—supporting team-based primary care for approximately 500,000 people annually, many from underserved communities.

Wider use of these tools has strong potential to improve care where it is most needed.
